A Green Sleeve is a plastic pocket from Alberta Health Services designed to hold your critical advance care planning forms.
Personal Directive: Your Green Sleeve comes with a blank personal directive form. If you already have a personal directive, replace the blank form with your own.
Goals of Care Designation (GCD): The Green Sleeve includes a blank GCD order form for your healthcare provider (doctor or nurse practitioner) to complete. If you think you might need a GCD order, bring the blank form to your healthcare provider for discussion. If your provider has already filled out your GCD order, replace the blank form with your completed GCD order, placing it as the first page in your Green Sleeve.
Advance Care Planning/Goals of Care Designation Tracking Record: This tracking record, included with your Green Sleeve, is for your healthcare provider to document conversations about your advance care plan and GCD. Keep it in your Green Sleeve for reference by all your healthcare providers.

Your Green Sleeve accompanies you through Alberta’s healthcare system
In an emergency, healthcare providers can quickly access your Green Sleeve to understand your healthcare wishes. Ensure it contains only the most up-to-date documents to avoid any confusion about your current wishes. Healthcare providers, including those in hospitals, may ask if you have a Green Sleeve.
Where Can I Get a Green Sleeve?
There are two ways to obtain a Green Sleeve:
- Ask your healthcare provider: Your family doctor or other healthcare providers can provide you with a Green Sleeve.
- Order online: You can order up to two Green Sleeves to be mailed to your home by following online instructions in the PDF below. Delivery typically takes 5 to 10 business days. If you need additional Green Sleeves (up to two per household) or have questions about your order, email [email protected].

Where Should I Keep My Green Sleeve?
Your Green Sleeve belongs to you. At home, store it in an easily accessible location, such as on or near your fridge. Emergency responders, like paramedics, are trained to look for personal medical information in this area. When you go to the hospital or a healthcare appointment, take your Green Sleeve with you and bring it back home afterwards.
