PROCEDURES FOR REGISTRATION using a cheque
- Lifetime membership is $40 per adult (GST included).
- Children of members are covered until they turn 18.
- Download, print, and fill out the membership application form. If you can't download the form, provide the name, address, phone number, and email address for each new member. Unique email addresses are preferred for each member's login.
- Mail your information with a cheque to: Cooperative Memorial Society, Suite 204a-223 12 Ave SW, Calgary, AB T2R 0G9.
