(effective January 1, 2023 - December 31, 2025) Plan prices exclude cemetery fees, permits, casket, cremation tray, out-of-town travel, and GST. A $35 CMS administration fee applies at purchase. In case of discrepancies, the contract terms prevail. When choosing a CMS price plan, consider any additional services you may need, as these may not be included.
