MEMBERSHIP APPLICATION
The Cooperative Memorial Society (CMS) helps people plan their funeral in advance. This makes things easier for your family and can save thousands of dollars in funeral costs. CMS is a not-for-profit group. We work with trusted, locally owned funeral homes across Alberta. When you join CMS, you get access to lower-cost funeral plans and help with making your wishes known.

To complete your membership, fill out the form. Beginning November 1, 2025, the Cooperative Memorial Society will begin collecting GST in addition to our $40 membership fee and $35 record fee. Until now, CMS has been paying GST out of these amounts, which reduced our operating funds. Going forward, GST will be added on top — $42.00 for memberships and $36.75 for record fees — to ensure fairness, compliance, and continued affordable service to members.
At the same time, a 4% processing fee will apply to all online payments made by credit card or Visa Debit. This fee covers the cost charged by our payment processor and helps CMS maintain low membership rates for everyone. You can avoid this fee by mailing a cheque along with a membership application. Memberships do not take effect until payment is successfully processed.
